Replacement I.D.'s

Lost, stolen or damaged cards should be reported to the University ID Card Office. If an ID card is lost, the established replacement fees will be charged and a new ID card processed. Please be advised that temporary ID cards are no longer issued.

The video imaging system has the capability to store images to be used for purposes other than the ID card (i.e. class rosters, yearbook, brochures, etc.) At the time of photographing, students will be asked to sign a waiver form indicating whether or not Slippery Rock University may reproduce the image for official university publications.
