Before your first day of work

Complete the following forms and return them to the Payroll Office before your first day of work:

• Student Employee Data Info
• IRS W-4
• Homeland Security I-9
  PHOTO ID IS REQUIRED FOR I-9
• LST Exemption Certificate 
  Include supporting documentation (i.e.: prior year’s W-2)
• Direct Deposit (optional but recommended)
